As a member of a interprofessional team, the Clinical Health Psychologist is responsible for the provision of behavioural health services for family practice clinics within the Primary Care Network, based on its Mission, Vision and Values. Responsible to develop and provide a full range of psychological programs and program evaluation services within the Primary Care Network, and to provide training and guidance to health coaches who are supporting patients identified to be at risk/early stages of chronic disease, to facilitate and enable lifestyle, cognitive and behavioural modifications that are realistic and maintainable. The Clinical Health Psychologist also provides specialty direct service (i.e. assessment, treatment planning, referral) for patients with complex needs.
