Family Practice Nurse-LPN (0.6 FTE -Perm)

POSITION SUMMARY The Family Practice Nurse (LPN) provides hands-on nursing care in a busy family practice clinic, supporting physicians and the care team to deliver safe, high-quality primary care.

The nurse assesses and supports patients with a range of conditions, provides education and guidance, and contributes to care planning and follow-up; duties vary by clinic and patient population.

Core responsibilities include delivering clinical nursing services (e.g., injections, wound care/dressing changes, suture removal, foot care, ear flushes, Pap tests) and providing chronic disease education and self-management support.

The role coordinates next steps based on patient concerns and test results, helps connect patients to local and regional programs, initiates appropriate investigations, maintains accurate documentation, and supports efficient patient flow while assisting physicians as needed.
