Care Coordinator / Discharge Planner
Role Summary
The Care Coordinator / Discharge Planner serves as a liaison between inpatient and outpatient care settings to ensure timely, safe, and effective transitions across the continuum of care. This role focuses on discharge planning, care coordination, patient outreach, and connecting members with appropriate clinical and community resources.
Use of this job is limited to the MMM business unit. HRBP consultation with Compensation is required before assigning this job.
Work Location & Schedule
Hybrid role based in San Juan, Puerto Rico.
Monday–Friday, 8:00 AM – 5:00 PM.
On‑site presence required at least 3 days per week, or as needed to support care coordination, patient outreach, and population health initiatives.
