Vancouver Coastal Health is looking for an Occupational Therapist or Physiotherapist Case Manager to join the Supported Housing team working out of Pacific Spirit Community Health Centre and supporting other facilities across Vancouver, BC.
An exciting opportunity to join a dedicated multi-disciplinary team of Case Managers, working with people living with complex medical and cognitive needs in Supported Housing sites, or at home with Choices in Supports for Independent Living (CSIL) funding. This position is embedded in the home health team and works as part of a small group of focused clinicians to support this population city wide, reporting to the Complex Rehab Supports & Supported Housing leadership team.
