Responsibilities and duties:
Client Care Functions:
Assess clients to identify issues and rehabilitation needs. Set client goals, develop, and document care plans. Request physician referrals and treat clients per their plans. Provide and recommend adaptive equipment and training for personal care. Offer counseling on lifestyle adjustments. Reassess and modify treatment plans as needed. Participate in team conferences and maintain communication with the care team. 2. Administrative Functions:
Maintain client statistics and attend meetings. Prepare and monitor the annual budget, order supplies. Develop and update policies and procedures. Supervise employees, volunteers, and rehabilitation HCAs. Ensure a safe working environment, conduct inspections, and report hazards. 3. Education Function:
