Registered Nurse (Primary Care Navigation Clinic - Integrated Care)

OVERVIEW

The Registered Nurse, as part of the Primary Care Navigation Clinic team, is expected to work in an interdisciplinary team to deliver high-quality primary health care services for optimal individual and community health outcomes. We are seeking a Community Health RN who is committed to client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to comprehensive health care.

The Registered Nurse in the clinic coordinates and directs patient care in compliance with the standards of nursing practice. The RN possesses a defined body of knowledge, clinical skills, and procedural skills and professional attitudes, which are directed to effective patient and family-centered care.
